Our Services
Person-centered support designed around each individual's unique goals, strengths, and needs — empowering adults with developmental disabilities to live with independence, dignity, and purpose.
How We Support You
HookMann Services provides three core service programs through California's Regional Center system. Each program is individualized, person-centered, and built around what matters most to the individual and their family.
Coordinated Family Support
Individualized support for adults living in the family home.
CFS provides individualized support to adults age 18 and older who receive Regional Center services and choose to live in the family home. HookMann Services works alongside individuals and their families to identify needs, coordinate services and resources, build skills, overcome barriers, access their communities, and support greater independence.
Support may also include assistance with scheduling services and appointments, transportation planning, identifying backup supports, accessing generic and community resources, and planning for future needs — including planning for individuals who may live with aging caregivers. Our approach respects each individual's choices, preferences, language, and culture.
What This Includes
- Identifying and coordinating services and supports
- Skill development and building greater independence
- Access to generic and community resources
